Assessment

Every resident at Heavenly Comfort AFC is unique. During an assessment, we’ll determine the level of care you or your loved one needs. We’ll begin to create a plan that targets their needs and gets them started on the path to achieving their goals and personal growth.

Case Management &
Support Coordination

We coordinate mental health and community care services between our team, professionals, and the resident’s family to ensure their needs are met. We are here to help our residents face new challenges head-on, keep them on track to their personal success, and help them access the appropriate services for their needs.

Medication Administration

Medication can play a key role in many of our residents’ care plans. We administer medication to our residents and monitor for side effects and changes in behavior. We work closely with health care professionals to continually meet the needs of every resident.

Support Independent Living

Teaching and encouraging our residents to be independent and do things for themselves is at the heart of everything we do. For many of our residents, our hope is to one day transition them into supported independent living.
